Asymptomatic left ventricular systolic dysfunction in hypertensive patients with disturbed ventriculo arterial coupling

Background Left ventricular (LV) ejection fraction (LVEF) is most commonly used validated echocardiographic parameter systolic function estimation, but left global longitudinal strain (GLS) an early indicator subclinical cardiac dysfunction, even when LVEF normal. Purpose To investigate ventriculo – arterial coupling (VAC) and LV in asymptomatic hypertensive patients without target organ damage to compare different parameters for function. Methods A total 110 (56 ± 14 years) with hypertension, were separated two groups: 32(men, ( 59 %)) normal AE/Ees ratio (Arterial elastance (AE) (Ees)) 78 (men, 64%)) decrease ratio, marker coupling. All underwent standard dimentional echocardiography Speckle tracking analysis - GLS. End pressure was determined from the brachial pulse wave. Arterial (Ees) calculated as pressure/stroke volume end pressure/end volume. Results Patients disturbed VAC had concentric hypertrophy whereas remodeling. There no significant differences EF, GLS all reduced. statistically diastolic parameters. Statistically found comparison (TABL. 1), are negative correlation between (r = -0.327, p 0.001) positive Ees (r=0.250, 0.015). Conclusion a more sensitive than LVEF, present patients. Further investigation needed confirm superiority over estimation its implementation clinical practice.
